This form asks for your name, contact details and what you are interested in, plus your PAN and date of birth - used only so Anees can check your mutual fund KYC status on the official platform, with your consent. No documents are uploaded on the form, and submitting it does not itself validate your KYC. Responses are stored in Google Forms and the linked Google Sheet, restricted to Anees (the form owner).
